As competition and regulations incite significant change among players in the healthcare industry, service providers are stepping up with capabilities and enabling technologies to move to an “as a service” approach. It will require a new level of collaboration between buyers and providers to reach new heights and effectively engage and manage consumers and patients in a more proactive and more personalized way through “population health management.”
Population Health Management Services are those that enable health payers, providers, public sector organizations, and new emerging entities, to focus on the health of the consumer – to identify their most relevant needs, engage them proactively where the propensity to respond is highest, support them through acute and chronic disease, and do so with financial acuity. Advances in data management and analytics, security, and cloud-based technology in particular are creating new opportunities for business process services providers and buyers to partner in this industry and drive up medical outcomes, and drive down costs.
In this inaugural Population Health Management Services Blueprint, HfS will take a look at service providers and how they are evolving their services and capabilities to help health plans, providers, and integrated health organizations to increase the effectiveness of healthcare across the care continuum.
